Print out the Veterinary Referral Form and give to your Veterinary Surgeon for completion and signing (some vets may already have copies of these forms)
- Your Vet will fax the referral form to us
- Hydro Hounds will call you to arrange an appointment
Once an appointment time has been confirmed, a welcome pack full of useful information will be sent to you including a copy of our terms and conditions for you to sign and bring with you to your first session.
Alternatively if you are interested in beginning hydrotherapy for your dog, you can contact us direct and we will endevour to arrange the veterinary referral form on your behalf.
The completed referral form is essential to beginning hydrotherapy treatment for your dog. We regret that we will not be able to treat your dog without it.
