Referral Form
Click the link below to download Hydro Hounds referral form and follow these three simple steps :
- Print the Veterinary Referral Form and give to your Veterinary Surgeon for completion and signing (some vets may already have copies of these forms)
- Your vet will fax the referral form to us.
- Hydro Hounds will call you to arrange your initial assessment.
The completed referral form is essential to beginning hydrotherapy treatment for your dog. We regret that we will not be able to treat your dog without it.
